Agency Selection
Step 1: Gather Agency Information
Agencies must first apply to participate in the Clinical Supervision Grant Program.
Information Required
You will be asked to provide:
Clinical supervisor credentials
Number of clinicians receiving supervision
Licensure track (Only LMSW’s are accepted at this time)
Agency populations served & treatment models
Agency annual revenue and Tax ID (EIN #)
Employee retention & recruitment strategies
Agencies are only eligible to receive funding through ONE source.
Step 2: Application Review
Applications are reviewed to ensure Supervisors meet licensing board requirements and Clinicians are eligible for supervision toward independent licensure. Agencies will receive notification once their application has been approved.

An email will be sent to your agency contact to notify of approval.
Gather Supervisor & Clinician Info
Supervisors AND Non-Independent Clinicians will need to complete an intake process.
